Using the online contact forms

Note: Mailboxes receiving submitted contact forms are not monitored when our offices are closed. This includes over the weekend, on public holidays and during the Christmas/New Year period.

Please call your local office for details on how to contact staff on call for queries requiring an urgent public health response.

To email a staff member, place a dot between their first name and surname, and then add @cdhb.health.nz (for example joe.bloggs@cdhb.health.nz). Please note that some staff may also have a number after their surname.

Note: NPHS Te Mana Ora staff are slowly and progressively moving to Te Whatu Ora email addresses (@tewhatuora.govt.nz – for example joe.bloggs@tewhatuora.govt.nz). You should receive an out-office message indicating when a  staff member has changed their email address.

Contact your nearest office to speak to a staff member who may be help with your situation or answer your question.

Te Mana Ora | Community and Public Health is part of the National Public Health Service

The New Zealand health system transition began on 1st July 2022.

Te Mana Ora | Community and Public Health is now part of the new National Public Health Service (NPHS) for Te Waipounamu within Health New Zealand. Our staff will continue to provide the same services until further notice.

The Public Health Nursing Service will become part of the National Public Health Service in the future. A date for this transition is yet to be confirmed.

Visit the Health New Zealand Ora Waitaha Canterbury website for information on other divisions or departments (formerly the Canterbury District Health Board), including the Public Health Nursing Service, and School and Community Dental Services.

Community and Public Health stopped providing an Elderly Falls Prevention programme from 1st January 2009.
