Disease Investigation Privacy Statement

The information provided and collected during an infectious disease investigation interview is confidential.

Your information will be stored on a secure national database.

It can only be access by people involved with your follow-up and national disease surveillance.

We may need to share information with anyone identified that you have been in contact with who requires follow-up – so they can help us decide if they are at risk of infection. This may include your name and locations visited while infectious.

We may also need to share health information with your GP (if you have one in New Zealand) to get the necessary support you require.

We can share your information to prevent or lessen a serious threat to public health if deemed absolutely necessary – under the Health Act 1956 (Subpart 5) and the Health Information Privacy Code 1994. For example to ensure that your contacts receive timely advice on testing and isolation requirements.

 

If you wish, you can access your health information. You can ask for it to be corrected if you think it is wrong or misleading.

You can direct any concerns about the privacy of your health information to Te Whatu Ora Privacy Officer or the Office of the Privacy Commissioner.

Distribution of Resources Disclaimer

Te Mana Ora | Community and Public Health is a regional Authorised Provider of Ministry of Health resources. We provide resources to Canterbury, South Canterbury, and the West Coast of the South Island, as well as the Chatham Islands.

Contact your local Ministry of Health Authorised Provider for resources or information if you live in another region of New Zealand.

Copies of Ministry of Health and Te Whatu Ora Health Promotion resources are not available for distribution outside New Zealand.

Currently the maximum resource quantity for online orders is 100 for any single item.
